3. Photocopy of birth certificate (if unmarried);

4. Photocopy of marriage contract;

5. Photocopy of checklist of course;

6. 1 pc. 2x2 colored picture (any background color)

7. 1 pc. short ordinary folder;

8. Photocopy of Certification of Employment of the employee form the

Human Resource Management Office Director.

 Describe fund generation scheme for scholarships and grants.

The office of Student Affairs also aims generate funds for students who

wants to be scholar through linkages of the agencies having the following types

of scholarship programs: CvSu State Scholarship, Academic Scholarship,

Entrance/Honorific, Students Assistantship, Job Experience Program, Employee

Dependent, Service Scholarship, RA 7160, PVAO Government, PD 577, DOST,

DND CHED PASUC, Cong. BoyingRemulla Scholarship, Cavite Provincial

Scholarship, KGMA Scholarship in the province of Cavite.

The Office of Student Affairs also have linkages through Private

Scholarship programs such as: Hawwel Chemical, Inc. Indang Community Group

of Northern California, Mrs. Netia Vida Obong and Teofilo Vida Jr. Scholarship.

PAGCOR, JR Remulla Scholarship Foundation, Jollibee Food Corp. Scholarship

Foundation, Phil. Rural Reconstruction Movement (PRRM), Gen. Mission

Scholarship Grant, Rotary Club of Tagaytay and PoncianoTuafio Scholarship.
