  Disability Coverage
Corizon Health’s disability coverage provides you with benefits if an injury or illness prevents you from working for an extended period.
Short-Term Disability Coverage*
Short-term disability is a voluntary benefit for active employees1 that replaces a portion of your monthly salary for
up to 180 days due to a non-work-related illness or injury. It pays a benefit of 60% of your weekly earnings (not to exceed $1,500 per week), but you must be actively at work on your benefit effective date for this coverage to begin. If you don’t enroll when you are first eligible, you will be required to submit an Evidence of Insurability (EOI) form and be approved for the benefit.
You must be unable to work for 14 consecutive days before becoming eligible for short-term disability, subject to medical determination review. Note: Pre-existing conditions may apply.
Long-Term Disability Coverage
Long-Term Disability (LTD) benefits are offered to all employees. LTD benefits begin after you have been unable to work for more than 180 days.
