to choose from a variety of benefits that can make a real difference in your life. We offer a broad range of benefits, including health care, life insurance, disability insurance, and much more. You can customize a benefits program that’s right for your personal situation.
This guide provides a summary of your benefit options. Please review it carefully before making your elections. All elections you make during annual enrollment will be effective January 1, 2019. If you are a new hire and in a benefits-eligible position, all benefits will be effective the first of the month, following 60 days of employment. After annual enrollment, no changes will be allowed unless you have a Qualified Life Event (such as a birth, death, divorce, marriage, etc.), and you request those changes within 30 days of the qualified event. You can learn more about Qualified Life Events on page 4.

Benefit Costs
Corizon Health pays the full cost of many of your benefits. For some of the other benefits, both Corizon Health and you share the cost or you pay the full cost. Premiums for your medical, dental and vision benefits are taken on a pretax basis. This means the cost comes out of your pay before taxes are deducted. When deductions are taken on an after-tax basis, this means your cost comes out of your pay after taxes are deducted. Below is a chart that indicates who contributes to the cost of each plan and indicates the tax treatment for payroll deductions.
