  Who’s Eligible to Participate
You are eligible for medical, dental, vision, life, accidental death and dismemberment insurance, and disability coverage if you regularly work at least 30 hours per week. If you are considered a full-time equivalent employee under ACA, you are eligible for medical insurance only.
Your effective date of coverage is the first day of the month following 60 days of employment in a benefits- eligible position. An enrollment window is provided each year so you can review and change your coverage
(if necessary) for the next plan year beginning January 1.
In addition, you may cover eligible dependents under
the plans offered by Corizon Health. You will be required to provide copies of eligibility documents to enroll your eligible dependents. If the required documents are not received within 30 days of the effective date of coverage, your eligible dependents will be dropped.
Examples of documentation include:
ƒ A legal marriage certificate (spouse),
ƒ A birth certificate (children), or
ƒ A completed Corizon Health Domestic Partner Affidavit. Note that this will be the last year that Corizon Health offers Domestic Partner coverage.
Rehires: You will be automatically reinstated to the benefits you had at the time of separation and those benefits will be effective the first of the month after your rehire date.
Dependents are:
ƒ Your spouse. The term “spouse” shall mean the individual lawfully married to you.
ƒ Your same or opposite sex domestic partner.
ƒ Any child of yours who is:
– Less than 26 years old; or
– 26 years old or older, permanently and totally disabled, living with you, and receiving over
one-half of his or her support from you. A child is “permanently and totally disabled” if he or she is unable to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment that can be expected to result in death or that has lasted or can be expected to last for a continuous period of not less than 12 months.
The term “child” includes:
ƒ Your natural child;
ƒ A stepchild;
ƒ Your legally adopted child;
ƒ A child placed with you for adoption;
ƒ A child related to you by blood or marriage if you are the child’s legal guardian as appointed by a court.
Note: Children of a domestic partner are not considered eligible dependents and cannot be enrolled.
How to Enroll: New Hires
Go to https://corizonhealth.benefitsnow.com to learn about the benefit choices available to you and to use the decision support tools to help you choose your benefits.
ƒ You can enroll online starting approximately 2 weeks after hire date
ƒ You will have a 45-day enrollment window beginning from date of hire
ƒ Coverage will be effective on the first of the month following 60-days of employment in a benefit-eligible position
ƒ See page 20 for specific enrollment steps for all employees

    Payroll Deductions
Deductions are taken each pay period, for the current pay period, not in advance. Deductions are not prorated; a full deduction will be taken for each pay period in which you are covered by benefits. If you do not receive a paycheck, you are still responsible for paying required monthly contributions to maintain your benefit coverage.
