Access to Personnel Files
Team Members have the opportunity to review their personnel file upon written request, once every six months, unless the Team Member is separated from the organization. The organization will comply with a request within seven working days if the personnel record is located within the state and within 14 working days if the record is located outside the state. A former Team Member may review the record once each year after separation for as long as the personnel records are maintained. The personnel record subject to review shall include job applications, wage or salary history, warning or discharge notices, fringe benefits information, attendance records, and performance evaluations. Other information such as letters of reference, results of employer testing, written comments of a personal nature about another Team Member or written comments made by and kept in sole possession of the Team Member's supervisor do not form part of the personnel record that is subject to review.
The record will be made available during the organization's normal hours of operation and at the Team Member's place of employment or other nearby location, in the presence of an organization representative.
Consistent with applicable law, a Team Member may have the opportunity to provide a written response, not to exceed five pages, to any disputed information in his/her personnel record.
After reviewing their files, Team Members may receive a copy of their personnel file upon written request. For more information, contact the Program Director.
