Changes in Personal Data
To aid you and/or your family in matters of personal emergency, we need to maintain up-to-date information.
Changes in name, address, telephone number, marital status, number of dependents or changes in next of kin and/or beneficiaries should be given to the Program Director promptly.
Care of Equipment
You are expected to demonstrate proper care when using the organization's property and equipment. No property may be removed from the premises without the proper authorization of management. If you lose, break or damage any property, report it to the Program Director at once.
