As stated earlier, the objective is to design systems and
processes to ensure that they reliably “close the loop” between
the ordering of tests or specialty referrals and the return of that
information back to the provider who ordered it and ultimately
to the patient. Effective systems can help reduce adverse events
and claims by decreasing reliance on memory or informal
mechanisms.
At a minimum, you should have a reliable tracking system in
place to track:
• Lab or diagnostic imaging results
• Referrals to other providers
• Missed or canceled appointments
• Previously identified abnormal conditions
• Test result notification and treatment follow-up
Many offices rely on the EHR tracking component for this
function. Many EHRs have ordering functions, reporting
functions, and/or tickler systems that track missed or canceled
patient appointments, missed test results, and/or referral
reports. Often, these systems will have additional opportunities
to ensure tracking is accomplished. For example, an EHR with
the capability to run reports of overdue results or outstanding
items can be used to identify any missed follow-up. In order for
the EHR system to effectively function, it is recommended the
practice develop follow-up, tickler, and tracking policies and
procedures and ensure the tracking system will close the loop
(from physician order to patient receipt of results).
