VOUCHER CREDIT REQUEST FORM
The Staff Development Committee honors hours of face-to-face or online professional development given by a
provider other than Huber Heights City Schools. Your out-of-district hours may be requested in ½ hour
increments. Upon approval and proof of participation, all hours will be credited toward earning your required
hours. Certified staff must earn at least 7 hours, classified staff must earn the total number of hours they work in
a day. Earning these hours allows personnel to be excused from Staff Development Day. Your hours will not be
“rounded up”– the entire 7 hours (certified) or your total number of hours you work in one day (classified)
must be earned from March 1 – February 28.
Directions: Please submit this form to your building principal in advance of the in-service for which you are
requesting voucher credit.
