As of September 1, 2015, I, Johnny Hernandez, Dentist, will terminate my service with
AZV, as is stipulated in my contract which terminates automatically when I turn 65.
However, I will continue working as a dentist t the same address,
providing service to private patients and private insurance companies.
All those currently registered under my name AZV will automatically be transferred to
John Hernandez, Dentist, at the same address (Savaneta 44, tel: 5847678).
Once the automatic transfer has taken place, patients may choose to any other AZV
dentist of their choice. AZV will have a Press Release with the information and
instructions how this procedure should be done.
