Heart Failure
VNSNY’s interdisciplinary care team works with heart failure
patients to manage their condition by utilizing the most
up-to-date research, evidence-based guidelines and our
years of experience caring for people with heart failure.
Depending on the patient’s individualized plan of care,
home care services may include nursing, physical therapy,
occupational therapy, speech therapy, social work, remote
monitoring services and a home health aide.
Our professional sta can help patients follow their doctor’s
recommendations and learn to manage heart failure at
home. Our services include:
• Helping patients to take medications properly and
monitor their e ects
• Helping patients to make necessary modi cations to
their diet to control their blood pressure, cholesterol,
and weight
• Teaching patients to recognize the signs and symptoms
that should prompt them to seek emergency care
• Teaching patients how to use a home blood pressure
monitor and keep an e ective log of their results
