VNSNY’s Remote Patient Monitoring (RPM) services
include utilizing the most advanced RPM technology and
communication to engage patients, families and caregivers
in the management of chronic conditions with the goal
of improving outcomes and reducing hospitalizations.

• Disease speci c engagement kits customized with
educational videos, care plans and medication reminders
• Virtual visit technology including the use of remote
assessment, video chat, wound imaging and secure
text messaging
• Tracking of vital health information 24 hours a day,
7 days a week
• Automatically transmit data to the RPM team and
communicate to the physician as needed
• An advanced nurse practitioner, who works in
conjunction with care managers
• Cell phone capacity with no LAN required
