Gender Affirmation Program
VNSNY’s Gender A rmation Program is designed to provide
specialized care to address the unique needs of each
transgender individual undergoing gender a rmation
surgery. VNSNY has developed clinical procedures and
policies speci c to this type of care, helping to ensure the
patient feels supported, engaged and independent.
With a team of over 200 clinicians, VNSNY:
• Assesses individuals before discharge
• Helps ensure a safe transition home
• Obtains insurance authorization for services
• Develops a personalized plan of care
• Provides skilled nursing services shortly after discharge,
as prescribed by MD
• Educates and promotes self-care
• Assesses and addresses mental health needs
