Behavioral Health
VNSNY o ers a specialized Behavioral Health program
designed for psychiatrically and/or functionally
homebound patients with anxiety, depression, dementia,
bipolar disorder, schizophrenia or other psychiatric
diagnoses. The program uses evidence-based screening
tools to provide a comprehensive assessment that guides
the creation of an individualized treatment plan. Through
an interdisciplinary team approach, patients receive:
• Care coordination
• Medication management
• Supportive counseling
• Community support
• Diagnosis-speci c psychoeducation, using
self-management tools to help improve adherence
Patients with Medicare and Medicaid (but not managed
care) are eligible for behavioral health services.
