VNSNY Medicaid Pending
This program is targeted to patients receiving skilled care
services from VNSNY. Patients who will need community-
based long-term care upon discharge, but who do not have
adequate nancial resources or insurance to cover this care,
may be eligible to receive assistance with the Medicaid
application process while on service with VNSNY. A VNSNY
medical social worker will educate the patient and family
about community-based long-term care options and assess
eligibility for Medicaid. If the patient appears eligible for
Medicaid, VNSNY will prepare a Medicaid application for
submission to the NYC Human Resources Administration.
Once the application is submitted, the patient can continue
to receive home care services under Medicaid pending
status. When the Medicaid is approved, the patient can
transition to a Medicaid Managed Long Term Care Plan.
VNSNY can also accept and review Medicaid applications
submitted by hospitals and nursing homes for patients who
are being referred for skilled care services. This can expedite
the Medicaid application process. If the patient appears
to be Medicaid eligible and the discharge plan is safe,
the patient can be admitted to VNSNY care on a Medicaid
pending basis.
