The Federal and NYS laws and regulations govern the
continuum of care:
1. Pre-Admission
2. Admission
3. Care Delivery
4. Discharge

Regulations: Pre-Admission

1. Initial patient visit shall be made within 24 hours of receipt and
acceptance of a community referral or return home from
institutional placement
2. Patient's health and supportive needs can be met safely and
adequately at home
3. Agency shall not be required to admit a patient when conditions
are known to exist in or around the home that would imminently
threaten the safety of VNSNY personnel
4. If the agency makes decision not to admit, MD shall be notified.
Refer to Adult Protective Services or other community services
as appropriate.
