1. RN must conduct an initial assessment visit to determine the
immediate care and support needs of the patient (unless
therapy only case)
2. Comprehensive Assessment at SOC: includes OASIS & VNSNY
questions and must be completed timely i.e. within 48 hours
(weekend, holiday exception) as per VNSNY P&P
(42 CFR 484.55; 42 CFR 484.10; 10 NYCRR 763.6; VNSNY P&P)
