In Conclusion
• VNSNY is involved in Value Based Care through both Medicare and Managed
Care efforts
• Our success relies on providing the right level of care to achieve patient outcomes
as our financial performance is tied to appropriately utilizing care while delivering

• To achieve success, we have to work with colleagues within our departments and
across departments to ensure that we provide the best care for our patients
