Types AND Time Points

 Start Of Care:

• VNSNY rule is to complete same day/ or early the next morning
of the SOC date

 Recertification:
• Oasis can be accessed by day 56, must be completed by day

 Resumption Of Care:

• Must be completed same day of inpatient facility discharge
 Transfer/Discharge/Death, or other Follow Up:

• Must be completed within 48 hours of becoming aware.

Who Can And Cannot Complete the OASIS

 Who CAN Complete the OASIS

Nursing:
• If RN is ordered, the RN completes the
SOC OASIS  Who CANNOT Complete
• All Resumption of Care Oasis (ROC) the OASIS
• Recertification Oasis
Physical Therapist: o LPN
• All ADR cases the Physical Therapist o PTA
completes SOC OASIS o COTA
• Recertification if RN is no longer on the o MSW
case
All Discharge Oasis (Discharge from Agency, o Aide
Transfer Discharge, and Transfer No Discharge)
can be completed by any clinician:
RN OT THE OASIS MUST BE COMPLETED BY ONE CLINICIAN
