OASIS Conventions (Rules)

 Care episode must have a beginning (SOC, ROC) and a
conclusion (D/C or Transfer)

 Day of assessment is the 24 hours preceding and
including the assessment visit

 Within one calendar day – means to the end of the next
calendar day

OASIS Conventions “facts to know”

 Direct observation is ALWAYS preferred
 Minimize the use of NA or Unknown
 Report the Ability, not performance or willingness
 “Caregiver does not impact the patient’s Ability to
perform a task.”

 Ability refers to the patient’s Ability to safely complete the
task
 Assistance is not limited to physical contact and includes
both verbal cues and supervision
