                              Braden Scale for Predicting Pressure Sore Risk

                                  •  Very High Risk: Total Score 9 or less
                                  •  High Risk: Total Score 10-12
                                  •  Moderate Risk: Total Score 13-14
                                  •  Mild Risk: Total Score 15-18
                                  •  No Risk: Total Score 19-23


                               If patient develops a pressure ulcer under our care, it is
                                 considered an adverse event

                              Surgical Wounds (Only Integumentary System)
                               All wound with drains even after drain removed
                               Central lines, implanted infusion, implanted venous access devices
                               Surgically repaired trauma wounds (repair of ruptured organs, muscle, fractures)
                               Sites from:
                                  •  Arthroscopy, mammosite incision,
                                  •  Muscle flap, skin graft donor site,
                                  •  Biopsy site, LVAD exit site,
                                  •  Orthopedic pin sites,
                                  •  Peritoneal dialysis catheter exit site
