The Blue
Card, will be
located in your
regional office,
and should be
utilized when
you attempt to
make a visit and
the patient is not
home.

Consent Form Packet
#1 is the single most important
form for the patient to
complete. We cannot start our
visit if the patient does not give
us consent.
The first four forms are to be
discussed with the patient.
They sign on Pointcare. The
last 3 forms are Medicare
Notices, to be discussed later.
