Risk Assessment : Braden Score

 Tell the patient / family / caregiver / MD the Braden score
and what it means (Low Risk is still Risk!)

 Advise the caregivers and patient “if they see something
(skin breakdown / stage 1 ulcer etc) to say something” to the
team because early identification of pressure related skin
damage can be treated with prevention measures
 The Stay healthy guide is a patient education tool available
on the intranet that incorporates those interventions as
a tool to use in your teaching and as a reference for the
patient and caregiver
 Order equipment as needed
 Bring all of VNSNY’s resources to the patient- PT/OT/ST/
SW/ HHA
