11. All patient information is communicated and documented. The clinician is responsible for
                       accurate, thorough documentation and comprehensive communication within
                       documentation standards and timelines established by VNSNY.

                   12. The clinician demonstrates effective case management skills; establishes priorities and
                       adherences to the visit frequency defined by the plan of care and/or dictated by the patient’s
                       need.  The clinician will assess the level of care needed by the patient and utilize the
                       assessment information, in collaboration with the physician and agency management staff, in
                       determining the daily/weekly visit schedules and supervision requirements.

                   13. Service is provided by the clinician in a quantity sufficient to meet agency goals including
                       productivity targets while maintaining quality of care.

               STAFFING/JOB DESCRIPTIONS


               Physical Therapist

               Physical Therapy services are provided by Physical Therapists licensed in the State of New York and
               by Physical Therapist Assistants (PTA) working under the supervision of a licensed Physical
               Therapist and within their scope of practice as outlined by the New York State Department of
               Education.


               Occupational Therapist

               Occupational Therapy services are provided by Occupational Therapists licensed/registered in the
               State of New York and by Certified Occupational Therapy Assistants (COTA) working under the
               supervision of a licensed/registered Occupational Therapist and within their scope of practice as
               outlined by the New York State Department of Education.

               Physical Therapy or Occupational Therapy services are

               provided under the client’s plan of care and include:


                   1.  Initial and ongoing assessments to determine the level of functioning including OASIS
                       assessments at appropriate points in time.
                   2.  Developing and revising the plan of care, in consultation with the physician and other members of
                       the care team.
                   3.  Providing therapeutic treatments and evaluation of equipment needs as applicable.
                   4.  Advising and consulting with the family and other agency personnel as applicable.
                   5.  Preparing clinical progress notes.
                   6.  Evaluating outcomes of care.
                   7.  Supervising Therapy Assistants and Home Health Aides as appropriate.
                   8.  Consultation and coordination of services with other disciplines.
                   9.  Participating in in-service programs.
                   10. Planning for discharge.
