RELIABILITY


               Prompt response to all messages is expected; any phone call (message) should be responded to
               within 1 hour. In addition, the regional office needs an accurate means of contacting each therapist;
               please be sure that all contact numbers are up-to-date.

               CASE STANDARDS


               Once you accept a case, VNSNY expects that you will follow-through with that patient by rendering
               high quality care according to MD orders and plan of care. If at any time, you feel that you cannot
               continue to provide care on a case, discuss it with your manager. Generally, each patient is seen by
               the same therapist to promote continuity and consistent care.


               TEAMWORK IN THE HOME

               Rehabilitation services are an integral part of a patient’s plan of care.  Therapist’s communication
               with all caregivers in the home is essential. All patients have a primary case manager who is either a
               nurse or a physical therapist. Regulatory standards require periodic communication from all
               disciplines.

               All cases will have a primary clinician assigned to them for every discipline ordered. This primary
               clinician is responsible for delivering visits on a consistent basis. For days that the primary clinician
               is unable to make visits, a covering clinician will be assigned and is responsible for coordinating care
               with the rest of the team.

               SUPPLIES

               Each regional office provides you with a bag, supplies for infection control measures (alcohol wipes,
               gloves, liquid soap and paper towels, etc.), thermometers and blood pressure cuffs and stethoscopes.  You
               will be shown the location and procedures for obtaining stocked supplies. Supplies are issued in quantities
               lasting 1– 2 weeks only.


               HOURS AND DAYS OF SERVICE


               The Visiting Nurse Service of New York is a 24 hour/day, 7 days/week, and 365 days/year agency.
               The office hours of operation are Monday through Friday from 8:00 A.M. to 5:00 P.M. The office is
               closed for eight major holidays: New Year’s Day, Martin Luther King Day, President’s Day,
               Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
               The Customer Care Center operates 24hours a day, 7 days a week to handle non-clinical calls from
               VNSNY patients and the general public.
