5.  Per Diem clinicians are paid for every signed visit note.

               Paid Time Off (PTO)


                   1.  All therapists and assistants, staff and per diem, are expected to request time off in advance.
                   2.  A request is submitted to your manager as soon as you are planning time away from patient care.
                   3.  All active patients should be identified for coverage and their individual clinical information,
                       including treatment schedule, plan of care, precautions, etc. are submitted to the manager.
                   4.  Every effort will be made to assign a covering therapist as soon as possible. No patient can be put
                       on “hold”; each patient must receive treatment according to their plan of care and MD orders.
                   5.  Be sure to discuss any coverage concerns with your manager prior to your time off.
                   6.  Staff clinicians submit their PTO request via WorkDay (to be discussed during orientation).

               TUITION REIMBURSEMENT

                   Eligibility Requirements
                   1.  Must be a full-time employee (working 36.25 hours/week for at least 6 months).
                   2.  To be reimbursed, passing grades must be obtained (according to school criteria).
                   3.  Must have active status (currently working; not on a leave of absence).

                   Policy
                   1.  Degree must be work related to VNSNY.
                   2.  VNSNY reimbursement is provided at 100% of the tuition costs for up to 18 credits.

                   Procedure
                       The Tuition Reimbursement Program procedure is all done electronically via HR Connect in
                       the VNSNY Intranet. Specific information about your school and degree will be needed, as
                       well as signed approval from your manager.



               CONTINUING EDUCATION

                   1.  Mandated for all home care field staff in accordance with licensing requirements.
                   2.  Any clinical education counts (with proof):
                         a.  Lecture presented at VNSNY.
                         b.  CEU course or matriculated degree course that is Rehab based.
                   3.  VNSNY presents several courses every year, free to VNSNY staff. CEU courses are
                       announced regularly via e-mail or you can log on to the VNSNY Learning Center.


               TRAVEL REIMBURSEMENT


                   1.  Staff is reimbursed for mileage, or mass transit using a MetroCard (for Manhattan
                       therapists). This process is done via WorkDay (to be discussed during orientation).
                   2.  Per Diem staff and contract therapists are not reimbursed for mileage or transit.
