VNSNY Tablet and Cellphone
The VNSNY tablet and cellphone will be issued to you before you see patients. The tablet is equipped
with wireless communication which can be used anywhere that cell phone reception is available. The
tablet and cell phone are to be used for VNSNY purposes only.
ESCORTS AND TRANSLATORS
1. Each region has escort and translator staff available; you can contact Contract
Administration at 212 609 7250 to request for Escort or Translator services.
2. All field staff can request an escort at any time in any area.
3. Many escort staff are bilingual and may serve as translators.
4. If the patient is not able to fully participate in the plan of care due to language barrier, the
Language Line 877-746-4674 can be used.
CASE ACCEPTANCE
Promised First Visit (PFV): The first PT, OT, ST visit is to be made
within 7 calendar days of the request for service as ordered in the POC, unless the
physician specifies otherwise.
There are several issues that may impact your ability to accept a case or meet the PFV:
a. Availability: When caseload is at maximum capacity, and there is no availability,
the case may be declined, and scheduling and assignments unit will call another
therapist.
b. Patient not available: Case has been accepted and the patient is unable to
be seen on the PFV date due to medical appointment, family request, or the patient
may request date or time change, etc. When able to accommodate such requests,
complete a Coordination of Care note documenting patient/family request and new
PFV date.
c. Case accepted/therapist becomes unavailable: Case is accepted,
and therapist is then unable to keep the commitment due to personal or professional
emergencies (e.g., sick day, transportation issues, emergency in field, etc.) The BSC
must be called to reassign the case to a therapist who can meet the PFV date. All
efforts should be made to honor the PFV
Case Manager
1. The Case Manager at VNSNY is a Nurse (RN), or a Physical Therapist (PT) if there is no
nurse on the case. All disciplines are part of the coordination of care in terms of services as
well as overall care.
2. The Case Manager should be contacted:
a. Within 48 hours of the initial evaluation to discuss POC and D/C plans.
