Click on the forms that will be reviewed with the patient.
After discussing and filling out the forms in your tablet, the patient/caregiver will sign only once for
all the forms:
If the patient is unable to sign and there is no caregiver present, select “caregiver” and you will
have a text box to explain the circumstances (e.g. Patient unable to sign d/t cognitive deficits,
daughter unavailable at this time, HHA not allowed to sign. Verbal consent obtained from
daughter to continue visit)
**NOTE: Once Electronic Forms is checked off, DO NOT go back into this section. The system
will recognize this action as you edit this section and will require the patient to sign again.
