The above named is a 38 year old known patient with end stage renal disease (ESRD) secondary to
hypertension, diagnosed with ESRD 4 months ago in University of Port-harcourt Teaching Hospital and

commenced haemodialysis.He is currently being managed on twice weekly sessions of haemodialysis,
twice weekly injections of subcutaneous erythropoietin and weekly injections of iron sucrose. He is
currently on tab exforge 80mg daily, tab astyfer 1 daily. Kidney transplantation is the definite treatment
option and will afford him better quality of life and life expectancy.
