                      Dr. Jason T. Hayes, MD, MBA, MSPH, FACP



                                                    Dr. Jason T. Hayes is a board certi ied internal medicine physician who will
                                                    be opening, in the summer of 2018, Personal Concierge MD ®, an of ice-
                                                    based concierge internal medicine practice. Located in the North Point area
                                                    of Alpharetta (GA), Personal Concierge MD O   will provide patients with
                                                    around the clock access to a personal care physician through of ice visit(s),
                                                    phone, text or email. Furthermore, the concierge practice will offer patients
                                                    same day or next day appointments, annual wellness exams, personalized
                                                    coaching, a subscription to “On Demand Health” newsletter and telemedicine
                                                    for simple cases such as strep throat, sinus infections and much more!

                                                    Background—Dr. Hayes is originally from Opelousas, a historic city in
                                                    Southwestern Louisiana. He graduated from Opelousas Senior High School,
                                                    in 1995, in the top 1% of his class.

                                                    Awarded with a full tuition scholarship, Dr. Hayes began his undergraduate
                                                    studies, in the fall of 1995, at Xavier University of Louisiana in New Orleans.
                                                    In the fall of 1998, he completed his studies and earned a Bachelor of Science
                                                    Degree in Chemistry with cum laude distinction.
                                                    Dr. Hayes continued his education, in the fall of 1999, at the historic Meharry
                                                    Medical College in Nashville, Tennessee. At this  ine institution, Dr. Hayes
                                                    won numerous awards for academic achievement and developed into an all-
          around student. Next, the student doctor was inducted, in the spring of 2003, into the illustrious Alpha Omega Alpha Honor
          Medical Society (A.O.A). Dr. Hayes graduated, in the spring of 2004, magna cum laude with dual degrees in both Medicine
          (M.D) & Public Health (M.S.P.H).
                 Training and Board Certi ication—In the summer of 2004, Dr. Hayes began his general internal medicine training
          at Baylor College of Medicine in Houston's world-renowned Texas Medical Center. While in residency, he provided top-notch
          medical care to patients while learning from the brightest minds in the Texas Medical Center. For his efforts, Dr. Hayes was
          awarded, in the spring of 2006, with the Texas Primary Care Resident Award. Subsequently, Dr. Hayes completed his general
          internal medicine training in the summer of 2007. Then, Dr. Hayes, in August 2007, became board certi ied through the Ameri-
          can Board of Internal Medicine (A.B.I.M.). This is a distinction that Dr. Hayes maintains to this day as he recently passed his
          Maintenance of Certi ication (M.O.C.) in December 2017.
                 Career—For the next 4 years, Dr. Hayes served as a National Health Service Corp physician in Memphis and Nash-
          ville, Tennessee. While caring for America's most underserved communities, Dr. Hayes gained many positive experiences dur-
          ing his service commitment including a leadership role as interim medical director. For his dedication to professionalism, in-
          tegrity and scholarship, Dr. Hayes was bestowed with the distinction as Fellow in the American College of Physicians (F.A.C.P.)
          in November 2009.
                 After completing his 4-year service commitment, Dr. Hayes gained various experiences in the public and private sec-
          tor over the next 5 years. He continued to serve in various physician leadership role(s) with the Tennessee Chapter of Ameri-
          can College of Physicians (T.A.C.P.).
                 Back to School—Sensing the need to enhance his skills, Dr. Hayes returned back to school in spring 2016. He attend-
          ed the Physician Executive M.B.A. (P.E.M.B.A.) program at the University of Tennessee Haslam College of Business in Knoxville,
          Tennessee. It is at this program that Dr. Hayes earned his M.B.A. and gained a multitude of business skills that have given him
          the foundation to open his own medical practice.

                 Interests—Although Dr. Hayes seems like a busy person, he does understand the importance of family and volunteer-
          ing. He cherishes his time with family and friends. Also, he supports organizations like the Drake House, an organization that
          provides a safe environment for single parent mothers and their children. Furthermore, Dr. Hayes is a member of the 100
          Black Men of North Metro, an organization that is dedicated to providing young men with valuable mentoring.
