The subcutaneous tissues, platysma and parotidauricularis
muscles are incised and blunt dissection is performed to
expose the sternocephalicus and sternohyoideus muscles.
Care must be taken to avoid damaging the jugular vein.
2 The sternocephalicus muscle and jugular vein are
retracted dorsally and the sternohyoid ventrally to expose
the laryngeal area.
3 Palpate the dorsal edge of the thyroid cartilage and incise the thyropharyngeus muscle along the dorsolateral
margin of the thyroid cartilage. Stay sutures (or Lone Star hooks) are placed to retract and rotate the thyroid
cartilage laterally.
4 The cricothyroid articulation is disarticulated to expose the cricoarytenoideus dorsalis muscle and the muscular
process of the arytenoid cartilage.
5 A stay suture is placed in the thyroid cartilage to aid in retraction. The cricoarytenoid articulation is disarticulated
and the arytenoid is dissected free from its attachments without entering the laryngeal lumen. Transection of the
sesamoid band (interarytenoid ligament) is optional.
