Heart Function Service: Heart Transplant Protocols
Shared Care Arrangements
Post-transplant many children and families prefer to have a shared care arrangement with their local
hospital and cardiology team. The arrangement will vary according to the medical needs of the child,
time post-transplant and the wishes of the family and local providers. Immunosuppression management
will be supervised by the Children’s Heart Transplant Team. The minimum a child will be seen at the
Transplant Center is for their annual cardiac catheter, biopsy, immunological testing and review.
