Heart Function Service: Heart Transplant Protocols

Post-transplant Follow-up: Infant (< 1yr age at transplant)

Day of Transplant – 2 months (POD 60)
1. Provider visits weekly
2. Echocardiogram and EKG weekly
3. CXR as needed
4. Routine labs weekly
5. CMV PCR at 2 weeks, one month and 2 months. IgG and IgM w PCR, if CMV R –
6. EBV PCR at 2 weeks, one month and 2 months
7. HLA antibody profile’s at 2 weeks, one month and 2 months
8. 0-3 mths Tacrolimus Level Goal (10-13)
9. CT scan at discharge if bicaval anastomosis

Month 2-4 Post Transplant (POD 120)
1. Provider visit every other week
2. Echocardiogram and EKG every 2 weeks
3. CXR as needed
4. Routine labs weekly
5. CMV PCR at 3 months. IgG and IgM with PCR if CMV R –
6. EBV PCR at 3 months
7. HLA antibody profile at 3 months
8. 3-6 mths Tacrolimus Level Goal (8-11)

Month 4-6 months Post Transplant
1. Provider visit monthly
2. Visit to include:
a. Echocardiogram and EKG
b. CXR as needed
c. Routine labs with each visit
d. CMV PCR at 6 months. IgG and IgM with PCR if CMV R –
e. EBV PCR at 6 months
f. HLA antibody profile at 6 months*
3. 3-6 mths Tacrolimus Level Goal (8-11)

Months 7-11
1. Provider visit monthly
2. Visit to include:
a. Echocardiogram and EKG
b. CXR as needed
c. Routine labs with each visit
d. CMV PCR monthly after 6 months. IgG and IgM with PCR if CMV R –
3. >6 mths Tacrolimus Goal (6-9)

Month 12 following transplant
1. Annual cardiac catheterization, biopsy and angiograms with ECHO, EKG, CXR
2. Transplant Annual labs
