Dear Dr.:
Your patient, , was listed for cardiac transplant as status ______ with UNOS. We would
appreciate you contacting one of the transplant coordinators if you are concerned that they have a
significant infection or a change in their clinical condition. They may be unable to receive a transplant
until they have improved or we may need to reevaluate their current outpatient treatment. We would
also ask that no live virus vaccinations be given specifically MMR, Varivax, Oral Polio, and Flu Mist.
