Outpatient Follow-up

Pre-transplant Outpatient Follow-up Protocols

Listed Status 1A: VAD Patient
 Provider outpatient visit twice weekly for 1 month then space to once weekly for 1 month, then
space every other week
 Routine labs weekly (BNP, CBC, Cr, BUN, and Lytes, LFTs)
 ECHO with provider visits
 EKG with provider visits
 HLA antibody profile every month
 Infectious work-up* see chart below
 CXR every 6 months

Listed Status 1B

 Provider outpatient visit weekly for 1 month then space to monthly
 Routine labs with each Provider visit (BNP, CBC, Cr, BUN, and Lytes. LFTs every other visit)
 ECHO 2 weeks post discharge, then at 1 month and every 2 months thereafter
 HLA antibody profile every 3 months
o For sensitized patients as per HLA recommendations (monthly)
o 2 week post any blood products
 Infectious work-up* see chart below
 EKG every 3 months
 CXR every 6 months
 Re-discuss (evaluation) each patient on a yearly basis

Listed Status 2
 Provider visit monthly for the first 3 months of listing and then can space to every 2 months if
stable
 Routine labs with each Provider visit (BNP, CBC, Cr, BUN, and Lytes. LFTs every other visit)
 ECHO every 4 months
 EKG every 6 months
 CXR every 6 months
 HLA antibody profile every 3 months
o For sensitized patients as per HLA recommendations
o 2 week post any blood products
 Infectious work-up* see chart below
 Rediscuss (evaluation) each patient on a yearly basis

Cardiac Catheterization
 Restrictive Cardiomyopathy- every 12 months and PRN
