Congratulations on your transition to aftercare! The aftercare program is often thought of as a graduate level of DBT, which means you have done some incredible work to get to this point in your journey. Throughout your treatment process, you have set goals, met them, adjusted them, and continued your growth to get to the point of graduation. Graduating a DBT program is a major accomplishment, and in aftercare we want to continue to support you in your life goals from here forward. This worksheet is designed to help you do just that: take the next steps toward reaching and maintaining your life vision goals.
