 VOLUNTARY BENEFITS
 Group Voluntary Critical Illness
Group Voluntary Critical Illness coverage is offered to you through AFLAC. This policy helps pay expenses that arise from the diagnosis of critical illnesses such as cancer, heart attack, stroke, or renal failure.
• $50 Health Screening Benefit – you may receive this benefit for one routine examination or other preventive testing once each calendar year.
• Spouse coverage is available. Spouses are limited to 50% of employee benefit amount.
• Benefit amounts available for $5,000 up to $50,000 for employees and $25,000 for spouse. Coverage is subject to underwriting approval. Each dependent child is covered at 50% of the primary insured’s amount at no additional charge.
• Guaranteed-issue coverage up to $30,000 for employee and up to $15,000 for spouse for new hires only.
Group Voluntary Accident
Group Voluntary Accident coverage is offered to you through AFLAC. This policy helps pay for out-of- pocket costs that arise from common accidents, such as fractures, dislocations, and lacerations. 24-hour coverage.
• $60 Health Screening Benefit – you may receive this benefit for one routine examination or other preventive testing once each calendar year.
• Spouse and dependent child coverage is available
• The plan provides 24-hour protection.
• There are benefits for inpatient and outpatient treatment of covered accidents.
• Coverage is guaranteed-issue - which means you qualify for coverage without having to answer health questions.
• Your premiums are paid through the convenience of payroll deduction.
• Coverage will be effective the date you sign the enrollment form.
• Your plan is portable (with certain stipulations). That means you may be able to take your coverage with you if you leave your job.
Group Voluntary Whole Life
Group Voluntary Whole Life coverage is offered to you through Texas Life. This policy helps provide additional income protection to your beneficiaries in the event of your death.
• Individual policies for spouse, children, and grandchildren without covering yourself.
• Provides income protection to your beneficiaries if you die.
• No physical examinations are required to apply. Coverage is subject to underwriting approval and Tier maximums.
• If you leave your job, you may take your policy with you and still pay the same premium.
• Provides an accidental death benefit.
• Builds cash value while your policy is in force.
Group Voluntary Short-Term Disability
Group Voluntary Short-Term Disability coverage is offered to you through AUL. This policy helps replace a portion of your income in the event you are unable to work due to an approved disability.
• Plan replaces up to 60% of income per month. Minimum $200 / Maximum $5,000 in $100 increments. Coverage is subject to underwriting approval. Choice of three elimination periods: 14, 30, or 90 consecutive days for a sickness and injury.
• Pre-existing conditions covered after 12 months.
• Maternity Coverage benefits will be paid the same
• as any other qualifying disability subject to any applicable pre-existing condition exclusion.
• Waiver of premium
• Your plan is portable (with certain stipulations).
• That means you may be able to take your coverage with you if you leave your job.
