Prescription Drugs*
1. Filling Your Prescription
Getting a prescription filled is an important part of your healthcare treatment. You can have two (2) fills of your prescription done at your local CVS or in-network pharmacy. After the first two fills, you will be required to fill your maintenance needs through the methods described in “maintenance medications”.
2. Maintenance Medications
Maintenance medications are drugs used to treat a chronic, long-term medical condition. You have two options for obtaining a 90-day supply of your maintenance medications:

Find Out How Your Prescriptions Will Be Covered
• Log on to www.caremark.com (you need to register before using this website) • Select the Understanding My Plan and Benefits tab
• Click on Drug & Coverage Cost
• Enter the name of the drug to look up coverage and cost information
4. Preventative Care: Generic Oral Type 2 Diabetic
Medication: Listing below covered at 100%
Doctors often don’t consider cost when prescribing medications. Type 2 Diabetic Drugs are a perfect example. Partnering with CVS / Caremark in 2019, Applied® will again be offering the following listed oral Type 2 Diabetes preventative medications at no co-pay. Ask your doctor about these lower cost prescriptions as a part of your treatment.
5. More Ways to Save on Your Prescriptions
• Utilize medications on the CVS / Caremark Preferred Drug List. Find out how your prescriptions will be covered: – Log onto www.caremark.com (you will need to register)
– Select understand my plan and benefits tab
– Click on drug & coverage cost
– Enter the name of the drug to look up coverage and cost information
• Talk to your doctor about available generic options
• Find discount cards offered by drug companies
– You can find it by asking your doctor for a card or you can search online for a discount card using the specific drug name
• Enroll in a Medical Flexible Spending Account to help offset the cost for your portion of the coinsurance
“Dispense as Written” Restriction
If you or your doctor chooses for you to receive a brand name drug when a generic drug is available (such as when the prescription contains a “dispense as written” restriction), you will be responsible for paying the difference between the cost of the brand name drug and the cost of the available generic drug. You will also be responsible for paying the applicable co-insurance amount for your prescription.

The first time you use mail order, you must complete the New Patient Information Form, available at www.caremark.com.
