Basic Life and AD&D Insurance

Your Life
1.5 times annual salary up to $100,000 for all eligible associates
An imputed cost of coverage (code on pay statement: GTLNEW) is added to your income and is subject to Medicare and Social Security taxes based on IRS regulation.
Voluntary Life Insurance
Coverage in $10,000 increments up to the lesser of five times annual salary (rounded to the next $10,000) or $500,000. Guaranteed Issue for Voluntary Life is the lesser of three times annual salary (rounded to the next $10,000) or $150,000***. Any elections over the Guaranteed Issue will require the completion of an Evidence of Insurability form
to be approved by Cigna.**

Spouse Life Insurance *
50% of associates coverage in $5,000 increments up to $250,000**
Child Life Insurance *
$2,000 increments up to $10,000 maximum

$2,000 increments up to $10,000 maximum
* The associate must elect voluntary life for himself or herself in order to elect spouse or child coverage
** If you elect coverage over the Guaranteed Issue, a link will be available for you to submit your Evidence of Insurability directly to Cigna for review and approval
