Supplemental Benefits for Medical Insurance
Applied® partners with Cigna to offer three types of voluntary benefits to help pay for expenses if you experience certain medical issues throughout the year. These benefits can be a valuable addition to a medical insurance plan in the case of an unexpected medical event that causes out of pocket expenses before you have reached your deductible.

Coverage
Description
After a 30 day waiting period, a cash benefit is paid directly to you for hospital admissions and confinement
Pays a lump sum cash benefit direct to you upon the first diagnosis of a covered condition. After the 30-day waiting period of coverage effective date, benefit percentage amounts are payable for covered conditions – see the list shown below. Associates may elect $10,000, $20,000 or $30,000 benefit amounts. Spouses are eligible for 50% of the associate coverage amount and dependent child(ren) are eligible for 25% of the associate coverage amount. Rates for this coverage are based on the age of each covered person annually and the use of tobacco products.
Another benefit of the Critical Illness Insurance is that it will pay $50 to each covered person for getting an eligible health screening test in the calendar year. Each covered person is limited to one benefit payment per calendar year. Examples of eligible health screenings are mammograms, colonoscopies, fasting blood glucose tests, cholesterol tests, blood tests for certain cancers, PSAs and more.

Pays a lump sum cash benefit direct to you for a broad range of off-the-job, accident treatments and conditions.
There are two plans with varying levels of benefit amounts. Each plan provides a cash benefit based on a stated scheduled amount defined in the policy, for certain hospital admissions, fractures, follow-up care performed
by physicians and physical therapy visits, burns and lacerations. Each injury type and severity must have been diagnosed and treated by a physician within a specified number of days. Benefits differ for residents of the State of Washington based on that State’s insurance laws.
The Accident Injury Insurance also has a wellness benefit that pays either $50 or $75, depending on the plan you elect, for the completion of certain wellness visits, health screening tests and preventive care services obtained. The benefit is payable once per calendar year per covered person after the 30-day waiting period from the effective date has been met.
*Please note, due to state insurance laws, this wellness benefit is not available to covered persons residing in CT, NH, NM, NY, ND and VT.
