60% of base wages and commissions up to a maximum of $10,000 per month less any allowed offsets. Benefit begins after 180 days of covered disability. Cigna Insurance Company currently provides our long-term disability coverage.
60% of base wages less taxes and other payroll deductions up to a maximum of $5,000 per month for days 15 to 180 of covered disability.**
** If you reside in the states of California, New Jersey, New York, Hawaii or Rhode Island, you also are covered by individual disability plans for your respective state (that may include a payroll disability tax deduction). The state benefits will be coordinated with Applied’s disability benefits.
