operations. If a self-provisioned solution is awarded, the applicant must light the fiber no
later than June 30 of the Erate program year. Additionally, if the awarded bid includes
special construction costs and you will not be able to light the fiber by June 30, you can
request an extension for special construction charges ONLY if the request is made prior
to June 30 of the program year.

130. Library Data - Square footage

Please provide the total square footage for your library or library system. Also attach a
schedule of your square footage for each library branch with a separate address. When
entering your information for your library, enter your library name or library system in
the header "library system. If you are a single site library, please enter the library name as
the library system. Libraries shall provide the total area for all floors, in square feet, of
each library outlet separately, including all areas enclosed by the outer walls of the
library outlet and occupied by the library, including those areas off-limits to the public.

197. Library Data - # of Patrons

Please provide a close estimate of the total number of patrons serviced by your library or
library system.

265. Mobile Internet Hot Spot Service

This question is only for mobile Internet hot spot service. Do NOT include data plans for
your cell phones on this question. Provide details on plans needed in the notes section.
Conditionally Eligible. Wireless hot spots will only be eligible in very limited cases
beginning FY2015. Applicants must prove that the individual wireless access plan
(mobile hot spot) is the most cost effective means of providing Internet Access. If the
applicant selects individual wireless access plans, they must retain the competitive
bidding documentation comparing wireless Internet Access solutions vs. individual
wireless access plan solution.
Off site use is NOT eligible and must be cost allocated. WARNING: E-Rate discounts
are NOT eligible for wireless Internet Access used off-site. Wireless Internet Access
MUST BE USED on school or library property for E-Rate discount,. WARNING- OFF
SITE USE IS NOT ELIGIBLE FOR E-RATE DISCOUNT AND MUST BE COST
ALLOCATED.

280. Data Connections

Include requests for your data connections between buildings (local area network,
campus area network and/or wide area network). The Second E-rate Modernization Order
in December 2015 (Effective for applications beginning July 1, 2016) allows applicants
to request funding for leased lit or dark connections, wireless or wired, third party
