reasonable effort to schedule the treatment so that it will not unduly disrupt Company
               operations, subject to the approval of your health care provider.

               Medical Certification

               If you are requesting leave because of your own or a covered relation’s serious health
               condition (including Service  member Family Leave),  you must  provide appropriate
               medical certification from the relevant health care provider within 15 calendar days after
               you request leave, if practicable.  You may obtain Medical Certification Forms from the
               Human Resources Department.  If you provide at least 30 days notice of medical leave,
               you should provide the medical certification before leave begins.  Failure to provide
               requested medical certification in a timely manner may be grounds for delay of leave
               and/or denial of FMLA/CFRA certification.

               The Company, at its  expense,  may require an  examination  by a second health care
               provider designated by the Company, if it reasonably doubts the medical  certification
               you initially provide for your own serious  health condition.  If the  second health care
               provider’s opinion conflicts with the  original medical certification, the Company,  at its
               expense, may require a third, mutually agreed upon health care provider to conduct an
               examination and provide a final and binding opinion.

               The Company may require subsequent medical recertification at the expiration of your
               previous certification.  Failure to provide requested recertification within 15 days, if such
               is practicable, may result in  delay of  further leave and/or  denial of FMLA/CFRA
               certification until it is provided.

               Any misrepresentation in connection with a leave request or disability will be grounds for
               termination of your employment.

               Leave Related to Military Service

               Military Exigency leave taken due to a “qualifying exigency” related to military service
               must be supported by a certification of its necessity.

               Service member Family Leave taken to care for a service member shall be supported
               by a certification by the service member’s health care provider.
               Reporting While on Leave

               If you take leave because of your own serious health condition or to care for a covered
               relation, you must contact the Company at least one week prior to your expected return
               date regarding the status of the condition  and your intention to return to work.  In
               addition, you must give notice as soon as practicable (within two (2) business days if
               feasible) if the dates of leave change, are extended, or initially were unknown.
