         Plan Administrators


         EBA&M
         EBA&M is our Benefit Plan Administrator responsible for providing administrative services to Burlingame Industries’ employees for
         the  health  care  and  prescription  programs.    EBA&M  is  contracted  to  act  on  your  behalf  in  administering  our  medical  and
         prescription benefit plans by:
         •   Issuing I.D. cards
         •   Maintaining eligibility
         •   Processing all claims and making claim payments to hospitals, doctors and facilities
         •   Answering your questions about how or when a claim is paid
         •   Answering detailed questions about your plan options and benefit coverage

                         Call (800) 249-8440 or go to www.ebam.com. You are encouraged to register online. Once you register and
                         log in to the secure site you can:
                         •   Check payment status of recently paid claims
                         •   Find detailed health plan information
                         •   Download claims forms




         Medical Insurance


         EBA&M (Anthem Blue Cross) | PPO Medical Plan
         With the Exclusive Provider Organization (EPO) plan, you may self-refer to any physician within the network. This plan requires that
         you only utilize doctors, clinics, and hospitals that belong to the network, except in the case of an emergency, however, you may
         self-refer to specialists. If you choose to see a provider who is not part of the network, you will be required to pay the full cost for
         your care.




                         Finding a Medical Provider
                         Call (800) 249-8440 or go to www.ebam.com. Select “Plan Participants” and “Find a Provider”.
                         •   California Employees: Select “Prudent Buyer Network (EPO and PPO Plans).”
                         •   Non-California Employees: Enter “Blue Card Network”.





         Summary of Benefits and Coverage (SBC)

         Health insurance issuers and group health plans are required to provide you with an easy-to-understand summary about your
         health plan’s benefits and coverage, referred to as a Summary of Benefits and Coverage (SBC). This guide is designed to help you
         understand  the  medical  plan  options  offered  to  you  by  Burlingame  Industries.  Please  refer  to  the  SBC  and  carrier  contracts
         provided by Anthem Blue Cross for additional plan details.
