Who May Enroll
All regular full‐time employees working at least 30 hours per week and their

eligible dependents may participate in Dynacraft’s benefits program.
Medical Insurance Your eligible dependents include:
- PPO Medical Plan 4 • Legally married spouse
- Tips for Using Your Medical Plan 5 • Registered domestic partners
Wisely • Children under age 26 regardless of student or marital status
Dental Insurance When You Can Enroll
Eligible employees may enroll at the following times:
- PPO Dental Plan 6
- Tips for Using Your Dental Plan Wisely 6 • As a new hire, you may participate in the company’s benefit plans on
the first day of the month following the completion of 1 month of full‐
time employment
Vision Insurance • During annual open enrollment
- PPO Vision Plan 7 • Within 31 days of a qualified change in family status as defined by the

Basic/Voluntary Life and AD&D 8 Paying For Your Coverage
Insurance Your contributions are deducted before taxes are withheld which saves you
tax dollars and resulting in more take‐home pay for you. The 2020 payroll
deduction amounts can be found on page 11 of this document.
Flexible Spending Accounts
- Health Care Spending Account 9 Please note that unless your domestic partner qualifies as your tax
- Dependent Care Spending Account 9 dependent for health care purposes under the Internal Revenue Code,
Federal Tax Law requires that the premium cost for your domestic partner
Resources and Contacts 10 and his/her children will be deducted from your paycheck on an after-tax
basis. The value of Dynacraft’s portion of the premium will result in
Employee Contributions 11 “Imputed Income” to you. This amount will be included as an additional
wage paid to you on your pay stub for the cost of the domestic partner’s
premium on which you will pay Federal Taxes.
Online Benefits Enrollment
With the EaseCentral Online Enrollment System, you and your family can access your benefits information whenever you want, from
home or any place where you have internet access. Use EaseCentral to view plan details, coverage amounts and costs.
To Enroll or Make Changes to Your Benefits
For optimal performance, it is recommended that you use Chrome or Firefox as your internet browser.
• Username & Password: Your initial login information will be sent via email from HR. Click on the LogIn
link and you will be prompted to create a password
• Website to login is https://dynacraft.ease.com
• If you forget your login information, please click on the “I’m unable to log in” and follow the prompt
Once you are logged into the website, follow the prompts on each page to complete your benefit enrollment. You will be asked to
verify that your personal information is correct and enter in any of your dependent information. If you have any questions, please
