ONLINE ENROLLMENT
With the UltiPro Online Enrollment System (Powered by PlanSource), you and your family can access your benefits information
whenever you want, from home or any place where you have internet access. Use UltiPro to make your benefit elections,
update your personal information, and to locate important benefits documents. Details on how to enroll in UltiPro will be
available on SharePoint (QSC’s intranet).
Documents and Forms
The Document Library in UltiPro allows you to download documents and forms such as Summary of Benefits and Coverage
(SBC), Annual Notices, Life Insurance Evidence of Insurability (EOI) forms, and much more.
It is very important that you log in to UltiPro periodically to verify your personal information and identify a life insurance
beneficiary.
