         VOLUNTARY BENEFITS


         AFLAC | PERSONAL ACCIDENT INDEMNITY

         Aflac’s Personal Accident Indemnity insurance will pay you a benefit to help cover your out-of-pocket medical costs in case of an
         accident. For covered accidental injuries, fixed benefits are paid directly to you, regardless of any other coverage. Benefits are paid
         according to a fixed schedule that includes benefits for x-rays, treatment by a physician, treatment received in a hospital room,
         follow-up treatment, and more. A wellness benefit is also included. Please note treatment must be received within 72 hours of an
         accident.

         AFLAC | PERSONAL CANCER
         Aflac’s Personal Cancer insurance can help you better cope financially and emotionally if a positive diagnosis of cancer ever occurs.
         Benefits  are  payable  for  the  first-occurrence/diagnosis  of  cancer,  hospital  confinements,  drugs/medicine  administered  in  the
         hospital, radiation and chemotherapy, bone marrow transplantation, stem cell transplantation, experimental treatments, surgical/
         anesthesia,  transportation,  lodging,  home  health  care,  extended-care  facility,  nursing  services,  hospice,  and  more.  A  cancer
         screening wellness benefit is also included.

         AFLAC | SPECIFIED HEALTH EVENT

         The Specified Health Event Protection plan provides hospital intensive care coverage for sickness and injury, and provides specified
         health event coverage for critical illness. Some benefits are payable for both hospital intensive care and specified health events -
         some benefits  apply only  to specified  health events. Please note, age reductions may  apply.  Covered  benefits include hospital
         confinement,  first-occurrence  benefits,  reoccurrence  benefits,  continuing  care  benefits,  transportation,  lodging,  benefits  for
         secondary specified health events, and more.

         AFLAC | HOSPITAL PROTECTION
         The Hospital Protection plan can help with deductibles, copayments, and other out-of-pocket costs associated with a hospital stay.
         This plan will pay a benefit in the event of a hospital confinement and rehabilitation. A mammography benefit is also payable when
         a charge is incurred for a mammogram.

         AFLAC | SHORT TERM DISABILITY
         The Short Term Disability plan provides a source of income if you are unable to work due to a covered illness or injury. This plan is
         sold on an individual basis, so you can choose the level of coverage that’s right for you. Aflac will pay you directly, regardless of any
         other  disability  insurance  benefits  you  may  have  including  Social  Security.  Aflac  provides  benefits  for  both  total  and  partial
         disability. Even if you’re able to work, partial disability benefits may be available to help compensate for lost income.

                        You may enroll for these voluntary AFLAC plans upon hire.  Otherwise, open enrollment with AFLAC is held in
                        October of each year.  At that time, you may add coverage or cancel your current plans with AFLAC.

         RETIREMENT PLANNING


         TRI-AD | 401(K)
         Saving for retirement can be a daunting task, so we have made this easier for you by offering a voluntary retirement savings plan
         called  a  401(k).  You  must  be  at  least  21  and  be  an  active  employee  for  at  least  60  days  in  order  to  participate.  This  plan  is
         administered  though  Tri-AD.  Once  you  are  eligible to  enroll,  if you  do  not  opt  out,  you  will  be  automatically  enrolled  at  a  3%
         contribution (this amount can be changed at any time).  If you choose to enroll, you determine your contribution amount.   There is
         not an open enrollment for the 401(k) plan.  So you may enroll, stop contributions, as well as make changes to your contribution
