Each of the benefits that you elect have an employee cost associated with them. We deduct these contributions conveniently
out of your paycheck. Your cost for coverage will vary depending on the option and level of coverage you choose. Your
medical, dental, and vision contributions are deducted before taxes are withheld, which saves you tax dollars. Basic Life
Insurance, AD&D and the Employee Assistance Program are provided to you at no charge.

The Affordable Care Act (ACA) requires Americans to be enrolled in medical coverage or pay a penalty when filing their taxes.
This requirement is referred to as the “Individual Mandate”. To meet the Individual Mandate requirement in 2018, you can:
• Enroll in a medical plan offered by United Pacific
• Enroll in coverage through a government sponsored program
• Purchase coverage though a health insurance marketplace
• Pay the tax penalty by not signing up for coverage
Since United Pacific’s medical plans are considered affordable and meet minimum actuarial benefit value (per the ACA
guidelines), eligible employees will not be eligible for a government subsidy through the marketplace, unless they meet
Medicare/Medicaid qualifications. For more information, please visit www.healthcare.gov.

Health Advocate is a program that provides you with experienced help to assist you in making your enrollment elections,
find in-network providers, and when dealing with benefit related issues. Their team includes highly trained registered nurses,
backed by a staff of medical directors and administrative experts. When you call Health Advocate for assistance you are
assigned a Personal Health Advocate who takes responsibility for helping you to resolve problems.
Here are a few of the ways that they help:
• Find in-network providers and assist in making plan election choices upon enrollment
• They help you get your claims and/or billing issues resolved
• They help you get answers to your medical questions about test results, treatment plans, and medications
recommended by your doctor
• They work with our insurance carriers to help obtain appropriate approval for services when necessary
• They assist in communications between your doctors and the insurance companies
• They help you locate and contact the best doctors, dentists, and treatment facilities in your networks
• They share unbiased health information when you need to make an informed decision about your care
Their years of experience make it easy. You need to make only one call or send an e-mail and your personal health advocate will:
• Act quickly and effectively on your behalf
• Protect your privacy and keep information strictly confidential
• Find the best answers
• Make any necessary follow-up arrangements
