Benefits
DISABILITY INSURANCE
Your chances of being disabled at some time during your working career are probably higher than you think. According to the Social
Security Administration report, a 20 year old at work has a 3-in-10 chance of becoming disabled before reaching retirement age.
Veritone has always believed that disability insurance is important because it replaces a portion of your income if you become
disabled and are no longer able to work. Veritone provides Short Term and Long Term Disability benefits to you through Anthem
Blue Cross as an eligible employee to provide income protection when an illness or non-work related injury keeps you from
working.
Anthem Blue Cross | Short Term Disability
Veritone offers you Short Term Disability (STD) income protection if you become disabled due to accident, sickness or pregnancy. If
you experience a temporary disability, benefits begin 8 days after the start of your disability and will continue up to a maximum of
25 weeks. STD works with state disability programs, Social Security, and any other group disability coverage, to provide you with a
combined monthly benefit equal to 60% of your pre‐disability earnings up to a maximum benefit of $3,462 per week.
Anthem Blue Cross| Long Term Disability
In addition to Short Term Disability, Veritone offers you Long Term Disability (LTD) income protection if you become disabled for an
extended period of time. If you become totally and permanently disabled, benefits begin 180 days after the start of your illness or
injury and will continue as long as you remain disabled up to Social Security Normal Retirement Age. LTD also works with state
disability programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal
to 60% of your pre‐disability earnings up to a maximum benefit of $15,000 per month.
Additional Services included for Long Term Disability:
• Vocational Rehabilitation—Testing and training, job modifications and job placement to help you return to active employment
• Social Security Assistance—Anthem will help you apply for social security while you are receiving LTD benefits and if necessary,
offer guidance through the appeal process.

You have the choice of having Veritone pay for your STD and/or LTD benefit or you can pay with post-tax employee contri-
butions. If Veritone pays, your benefit will be taxable income when you file a claim. If you pay, the benefit is not taxed.
