Employee Assistance Program

Anthem Blue Cross | Employee Assistance Plan (EAP)
The Employee Assistance Plan (EAP) through Anthem Blue Cross provides you and your household members with
free, confidential assistance to help with personal or professional problems that may interfere with work or family
responsibilities and obligations. Services are available 24 hours a day, 7 days a week via a toll-free nationwide
number. You and your household members can receive up to 3 counseling sessions face-to-face or online using
LiveHealth Online. Services are designed to help you and your family reduce stress, balance your work and family
responsibilities, and improve your quality of life.

Employee Assistance Program Features:

• Toll-free 24/7 consultations and referrals
• 24 hour crisis intervention
• 24 hour telephonic consultation with licensed behavioral health clinicians
• Referrals for up to 3 free, face-to-face counseling visits for each behavioral issue
• Referrals to community resources for localized support
• Telephonic consultation for legal and financial counseling
• Access to over 100 legal forms to help in situations like creating a power of attorney or a bill of sale
• Resources to help with question on creating a will
• Access to a fraud resolution specialist to help with identity theft victim recovery
• An online resource library with a variety of health and emotional wellbeing content

These resources and services are available in addition to the mental health benefits offered through your medical
plan.

may call (888) 209-7840 to be immediately connected to an EAP counselor.

Travel Assistance Services

Anthem Blue Cross | Travel Assistance Services
Incipio provides a Travel Assistance plan to you at no cost. This program, arranged by Anthem Blue Cross and
provided by Europe Assistance USA, offers emergency assistance when you and/or your family are traveling 100+
miles away from home or work. Travel Assistance is available 24 hours a day, 7 days a week with trained
professionals to provide valuable support in a medical emergency, such as locating doctors, dentists and facilities,
setting up emergency transfers, sending travel companions, pets, and vehicles home, coordinating bedside visits,
and setting up and paying for the return of mortal remains (see contract for restrictions). In addition, emergency
travel support services are available and provide services such as immunizations (shots), passport needs,
emergency payment and cash advances, emergency messages, and legal help.
