                Flexible Spending Accounts (Continued)


                You can use your FSA funds to pay for a variety of eligible expenses for you, your spouse, and your dependents. The
                IRS determines which expenses can be reimbursed by an FSA.  Below we have provided examples:

                Termination of FSA:
                Please note, that when an employee terminates employment, COBRA is typically offered on the Health Care FSA (if
                there are unspent funds).  If COBRA is not elected, you have 90 days from the date of termination to submit claims
                for reimbursement.

                To help better understand, please see example below:
                •   Employment termination date: 4/16/2019
                •   Health Care FSA remaining balance: $400
                •   Health Care FSA funds must be used by: 4/16/2019
                •   Time allowed to submit for claims incurred by 4/16/19: 90 days

                Rollover funds may be handled differently depending on your specific situation, so please contact Human Resources
                or Discovery Benefits if you have questions.


                Pet Discount Program


                United Pet Care (UPC) | Discount Program
                With the United Pet Care (UPC) discount program, you can reduce the cost of your veterinary bills by saving up to 50%
                on each veterinary visit for your pet when you visit a contracted UPC provider. Your premiums are paid through
                payroll deductions on an after-tax basis. This plan offers discounts for office visits, procedures, medications, and
                more. With UPC’s program, there are no deductibles, no claim forms, no exclusions due to pre-existing conditions or
                age, no annual dollar limitations, no waiting periods, and no limit on the number of visits annually. Two discount
                program options are available through UPC.

                               Note: Enrollment is a two step process, you will need to enroll in ADP as well as register with
                               United Pet Care directly (via the website or phone).  You may enroll in this program at any
